You must complete all the following steps to fill out the enrollment
form:
1. Provide the name and contact
information for your organizational
contact, and enter Organization and
Organization Unit.
2. Choose your language encoding and optionally, a
localized Organization and
Organizational Unit (initial administrator
enrollment only).
3. Provide the name and contact information for your Managed PKI administrator .
4. Enter a challenge phrase.
5. Choose the number of certificates you are buying.
6. Provide the name of and information about your Billing Contact.
7. Indicate your method of payment.
8. Read and agree to the ADACOM S.A. Managed PKI Agreement .

If you want to have separate
affiliations associated with your Organization, each one becomes
a separate "affiliation," and thus, a separate Managed PKI license.
Organizational Unit
The
Organizational Unit is an optional entry.
If your organization has more than one instance of Managed PKI, make sure to
enter a name here that can be easily distinguished and identified throughout
your organization.
Managed PKI Administrator
Enter the first administrator's information. This refers to the first Managed PKI administrator associated with this Organizational Unit. You can add additional administrators for an additional charge after you have successfully enrolled your first Managed PKI administrator. The additional administrators can help to manage this designated organization.
The first administrator has full privileges for managing certificates. Additional administrators have read-only access until they are given additional privileges by an administrator with full privileges.
For Managed PKI for SSL:

For Managed PKI for SSL Premium Edition:

Enter a Challenge Phrase that will be easy for you to remember and hard for someone else to guess. Write it down if necessary and keep it in a very secure place.
Important! Remember the challenge phrase. You will need this phrase to pick up your certificate and manage your service and user certificates.
Purchase Your Managed PKI User Certificates
Choose the initial number of end user certificates you will purchase. You can purchase additional user certificates at any time after setting up your service.
Billing Contact
The Billing Contact is the person responsible for payment and for notifying ADACOM S.A. of billing changes. Since you are filling in this enrollment form, you might be the Billing Contact, the person in your company, organization or government agency responsible for financial matters connected with the ADACOM S.A. Managed PKI service, and who is authorized to accept the Subscriber Agreement with ADACOM S.A. on behalf of your company/agency/organization.
Payment Method
Choose How You Will Pay for the service
You can pay for your Managed PKI service by credit
card, purchase order, or check.
Credit
Card
Choose the type of credit card you want to use, and enter
the credit card number in the provided field. Choose the expiration month and
year from the pull-down menu, and enter the card holder's name.
Purchase Order
If you pay by purchase order, enter your purchase order
number in the enrollment form. The Digital ID Center cannot process your request
until a hard copy of the purchase order is faxed to:
+30210-9577430. Make sure to reference your server/domain name
on the fax cover sheet.
Check
If you pay by check, include your check number in the enrollment form.
All
checks must be drawn on a US bank and in US funds.
The Digital ID Center cannot process your Digital ID until we receive your check
mailed to:

Accept the Subscriber Agreement
Read the Subscriber Agreement carefully. If you agree to the terms and conditions of the agreement, click Accept to continue enrolling. If you do not agree to the terms and conditions, click Decline.
After you accept the agreement, the enrollment is complete. If all the information you entered is correct, your technical and Organizational Contacts will receive an e-mail message confirming your enrollment. The message will include a URL to which you can go and check on the status of your service request.
When your service is approved, we will send the administrator instructions on how to pick up and install the Administrator ID and begin using your new Managed PKI service.
